How can I create a one-time order form to receive Paypal payments?

So here's the back story:

 

I'm a nurse at a children's hospital on the cancer unit. Today we found out that a longtime, beloved patient passed away. She LOVED to color and decorated every available surface on our unit. She also made us a binder of some of her coloring pages, sort of like a comic book with little quotes put in poking fun at some of the staff! As a way to remember her, we are going to scan the pages of the binder and make them into a photo book for any of the staff to keep and also to give to her family as a gift from us.

 

I'm heading the project. I'd love to set up a page with an order form so that people can select how many they want to order, add an option for shipping if it's someone not currently still working at the hospital who could pick it up from me directly, and then pay me through Paypal before I submit the order. There would likely be anywhere from a dozen to 50+ people who may submit orders, not sure of the level of interest yet.

 

Can anyone recommend an easy way to do this? I don't have a website to put a paypal button on and I don't know how to set up the order form. I want it to be straighforward and also make it easy for my coworkers to order, but I also want to ensure I get everyone's payment because I'm putting all the money upfront to order the books.
