Voided Check or Direct Deposit Form Needed

I need a Direct Deposit Form in order to set up direct deposit with my employer. I have the Routing Number and Account Number for my PayPal Mastercard, But since this is not a traditional checking account where I can actually give them a copy of a voided check, they are requesting an Direct Deposit Form from the bank. Normally, I would just go into the bank and ask for one and a teller or representative could print one for me with my account information on it as well as the bank name/routing number, etc. But since this is not a traditional account/bank I am not sure how I would get this form. Hopefully one exists or I will not be able to set up direct deposit. Help Appreciated!!!

I don't quite think the PayPal mastercard is setup for what you want to do with it.  If it is direct deposit and a card you need I would urge you to look at the Rush card as you can get that info tonight and hand it over to your employer tomorrow for HR to input.  As with any prepaid there are monthly fee's so I would look at them and fully understand the ins and outs.  Netspend used to be a reliable card for many but I have seen reviews of all but Rush Card kind of tank over time on top of the Rush Card you can get at many local outlets tonight.  If you aren't able to get a checking about because of being in Chex systems and just want a checking account have you considered googling banks that offer a second chance checking account?
