ACH = Automated Clearinghouse (electronic funds transfer mechanism) moves money from one bank to another.
Xfer = transfer
ACH Hold is for a period of time, after you deposit a check. Any or all of the amount may not be part of your usable balance to make sure check is legit and that the funds are collected from the check’s issuer.
Paypal Inst Xfer is an order via PayPal to debit funds from your account, normally a few days after the purchase if not instantly. If you have a split bill, purchased something for $20 and had $5.00 in your PayPal balance, for example. $15.00 from your bank account is deducted after emptying your PayPal balance a few days after. Check to see if any recent transactions suit the bill.
Keep in mind that the $15.00 transaction won’t have a separate entry in your PayPal account activity when it’s posted in your bank activity. Just in your bank account only. If you look up the original transaction for the purchase in PayPal account, it will mention the split payment in the “Paid by” section.

Yes, once the hold expires, for example say a purchase you made but it failed or was canceled, the funds would be released.
