How to add a shared credit card to my wife and mine's individual PayPal accounts

Do each of you have a card but each card in your own names associated with the same card account? I suggest you order a secondary card instead of putting the same card for both accounts. Call your card issuer to request a second card under your wife's name or your name, whoever will be the secondary card holder on the account.

Its best to have name on card to match name on PayPal account.

In other words, say, you are the main card holder of the account, then you can request a secondary card in the name of your wife. Or are both names on the one account? If the latter is the case, it still may make sense for each person to have a separate credit card in just his or her own name.

Thanks, our individual credit cards actually do have each of our names (my wife and me), but they share the same credit card number.  I finally got it resolved through PayPal phone support (and 40 minutes on hold wait).  PayPal security makes it tough to add the same number credit card to different accounts, we were all locked up and they had to manually assist.  Now all good.
