How to add surcharge to all products?

I have a product with several price options: basic, standard and premium. How can I add an option that adds a specified amount to each item, without having to create 3 additional options?

So if a user selects the standard option AND selects "Please do this for me" it adds $149?

Set the surcharge up as a separate listing.

The buyer buys the basic package and a surcharge listing.
