Need help applying for PayPal Prepaid Mastercard

Can someone tell me if I'm doing something wrong applying for a PayPal Prepaid Mastercard.  I went ti this URL a few weeks ago and filled out the very short form:

It says to expect the card to come in physical mail in 7 to 10 business days.  But it never arrived, and I got no email about it from Paypal to the email address I supplied in the form, the same one for my 17 year old Paypal account.

The form only asks for: First name, Last name, Street Address / Apt #, ZIP code. Email address.  No city.  I was 2 hours on the phone speaking with Paypal representatives today who forwarded me to several different divisions.  But none could tell me what the status of my application was, or help me apply again while on the phone.

Have other people been able to receive a prepaid card through this process successfully?  Does anyone know why this may be failing for me?

You could contact PayPal PrePaid and ask if they are having issues. I'll link their contact details below.

I also wanted to let you know that PayPal does have available the Cash debit card. This card is directly linked to your available PayPal balance and there are no transfers necessary. The card is no cost and there are no monthly fees. When you order the card, PayPal will enable some other helpful features in your PayPal account.

If you do receive the PrePaid card, you can have both it and the Cash card, if you want. The PayPal PrePaid card does have some helpful features that the Cash card doesn't offer.
